Pursuant to your signed contract, notice of disenrollment must be submitted at least 4 weeks in advance of the final date of enrollment.

This form must be completed for each child you are disenrolling and be delivered to the sunset administration. Verbally communicating to a staff member that you wish to disenroll does not constitute giving a formal notice.

Upon receiving this form, we will charge your tuition account for 4 weeks of tuition. Your account must be settled prior to your child’s final date of attendance.
